  • Funded separately from standard Support Coordination in your plan
  • Includes crisis planning, risk management, and multi-agency coordination
  • Works closely with clinical teams, housing providers, and family
  • Available for participants with psychosocial, physical, or intellectual disability

Understanding the Difference

Many participants confuse Support Coordination with Plan Management. Both are important — but they do different things.

Support Coordination

  • Helps you understand and implement your NDIS plan
  • Connects you with providers and services
  • Supports you to build your own capacity and independence
  • Monitors progress toward your goals
  • Prepares for and attends plan reviews with you
  • Funded from Capacity Building budget

Plan Management

  • Handles the financial administration of your NDIS plan
  • Pays your provider invoices on your behalf
  • Provides monthly budget statements and tracking
  • Allows you to use unregistered providers
  • Does NOT help choose providers or manage your supports
  • Funded from a separate Plan Management line item

Common Questions

Support Coordination must be included in your NDIS plan to access this service. If it's not currently in your plan, we can help you request it at your next plan review or through a plan variation request to the NDIA.
Yes. You have the right to choose any registered NDIS Support Coordination provider. You can switch providers at any time by updating your service agreement. We welcome participants who want to change.
This depends on your plan and your needs. Typically coordinators check in at minimum monthly, with more frequent contact during setup, plan transitions, or when issues arise. We always aim to be responsive and available.
Continuity of care is our priority. If your coordinator changes, we ensure a thorough handover and introduce you to your new coordinator before the transition. Your records, goals, and relationships are always preserved.
